(3) The DoD has formally identified the soldier who is listed as Duty Status Whereabouts Unknown (DUSTWUN) in Baghdad: Specialist Ahmed K. Altaie, 41. Altaie has been missing now since he was apparently kidnapped from the home of a relative on October 23rd.

(2) The DoD has also announced a new death that CENTCOM has not reported, a Marine who died on Wednesday, November 1st, from enemy action in Al Anbar Province: Lance Corporal Minhee Kim, 20, of Ann Arbor, Michigan.

Saturday, November 04, 2006 5:55 AM - The Fresno, California, Bee is reporting the death of a Modesto native in an IED attack in Baghdad on Thursday, October 2nd. Army Sergeant Joseph Gage, 28, was killed along with two other soldiers and an interpreter in the explosion. He was a member of the 101st Airborne Division out of Fort Campbell, Kentucky. He was also 11 days away from completing his second tour of duty in Iraq.

Saturday, November 04, 2006 10:59 AM - MySanAntonio.com is reporting the death of a San Antonio Marine reservist in Iraq. Lance Corporal Luke Benjamin Holler, 21, died on Thursday, November 2nd, when a roadside bomb struck his patrol. We are assuming for now that he is one of the three Regimental Combat Team 7 Marines previously reported by CENTCOM to have died from enemy action on that day.

Monday, November 06, 2006 5:25 AM - KTVA-11 in Anchorage, Alaska, is reporting the death of a Fort Wainwright, AK, soldier in a small arms fire attack in Baghdad on Saturday, November 4th: Specialist James Lee Bridges, 22, of Buhl, Idaho. Bridges was a member of the 172nd Stryker Brigade Combat Team that had its deployment extended at the last moment this past summer. Seven of their troops have now died since they were shifted to Baghdad to reinforce troops already stationed there.

(1) The British Ministry of Defense has announced the death of one of their soldiers in a small arms fire attack on a coalition base in Basrah on Monday, November 6th. The soldier was assigned to the 2nd Battalion, The Duke of Lancaster's Regiment, based in Catterick Garrison, North Yorkshire, England.
 
(2) CENTCOM is reporting the death of a soldier in northwest Baghdad from an improvised explosive device on Monday, November 6th.

(6) The Towanda, Pennsylvania, Daily & Sunday Review is reporting the identity of one of the two pilots of a helicopter that crashed in Salah ad Din Province on Monday, November 6th: Chief Warrant Officer 4 John Priestner, 42. Priesner grew up in the vicinity of Le Raysville, Pennsylvania, but had been living in Sanford, North Carolina. This was his third tour of duty in Iraq.

Wednesday, November 08, 2006 4:19 AM - The Columbia Missourian has provided more details on the death of Marine 2nd Lieutenant Mark C. Gelina on Saturday, November 4th. According to that source, Gelina was preparing a battle position when he fell from a rooftop. The death was classified as "non-hostile" by the military.

According to the article, Henderson had been married for all of three weeks before he was deployed to Iraq.

(1) The DoD has confirmed the death of Marine Lance Corporal Ryan T. McCaughn, 19, of Manchester, New Hampshire, from enemy action in Al Anbar Province on Tuesday, November 7th. This agrees with the date cited in media articles ... and raises the question: is this a new death not previously reported by CENTCOM? Or did CENTCOM err on their date for this release? His unit, the 1st Battalion of the 6th Marines, was definitely attached to the 1st Brigade, 1st Armored Division operating out of Ramadi. At any rate, his death date will be changed to the 7th in our database.

(2) The Associated Press is reporting the death of a 28 year old soldier from White Swan, Washington, in an IED and small arms fire attack in Baghdad. Sergeant Lucas T. White would appear to be part of the 172nd Stryker Brigade Combat Team who saw their deployment to Iraq extended and who were then assigned to beef up troop levels in Baghdad. His date of death is not given in the article. However, he could well be the Monday, November 6th, death described in this CENTCOM release. White was a member of the Confederated Tribes of the Umatilla Indian Reservation in Oregon.

(1) Reuters AlertNet is reporting the deaths of a Polish soldier and a Slovak soldier near the city of Al Kut in Wasit Province late on Friday night, November 10th. According to another report on English language Polskie Radio, insurgents detonated a remote controlled improvised explosive device under their vehicle as their convoy passed, then opened up on the vehicle with machine gun fire. Reuters says that a second Polish soldier and an Armenian soldier were wounded and flown to a military medical facility in Baghdad. The dead have been identified as:
    Sergeant Tomasz Murkowski of the 16th Pomeranian Mechanized Division out of Elblag, Poland
    Sergeant Rastislav Neplech of the Slovakian Army
(2) The Associated Press is reporting the death of Army Reserve Sergeant 1st Class Rudy Salcido, 31, who was raised in La Puente, California. The exact circumstances of this death are unknown, but he reportedly served out of the large base at Al Asad in Al Anbar province. Although the article states that he died on Wednesday, November 8th, it is entirely possible that it was Wednesday stateside, but Thursday the 9th in Iraq. This would make him a good candidate for the death described in this CENTCOM release.

Saturday, November 11, 2006 10:05 AM - The Polish Ministry of Defense has now confirmed the deaths of a Polish and a Slovakian soldier in a roadside bomb blast. According to that release, the deaths actually occurred early in the morning on November 11th, Iraq time (late in the evening, November 10th, Poland time). A rough translation of the beginning paragraphs of the statement is as follows:
 

A Polish and a Slovak soldier have perished in Iraq

 

Two Multi-National Division – Center South soldiers, a Pole and a Slovak, have perished at about the hour of 1 AM on 11 November (or 11 PM [23:00 hours] on 10 November Polish time) near Al Kut, Wasit Province.

 

The soldiers, who died as a result of an explosion, were Sierżant [Sergeant Class II] Tomasz Murkowski, 30 years old, of the 13th Anti-Aircraft Regiment based in Elblag, and Starszy Sierżant [Sergeant Class I] Rastislav Neplech. The wounded soldiers, a Pole and an Armenian, were transported to a hospital in Baghdad by helicopter. Their condition is stable. Families of the soldiers have been informed of the event.

(1) MNF-Iraq is reporting the deaths of three 1st Brigade, 1st Armored Division, soldiers from enemy action in Al Anbar Province on Saturday, November 11th.
 
(2) The British Ministry of Defense is reporting the deaths of four of its service personnel in an attack on one of its patrol boats on the Shatt Al-Arab waterway south of Basrah on Sunday, November 12th. In addition, three troops have been seriously wounded, one with potentially life-threatening injuries. The BBC provides further details in this article, stating that the attack was a very unusual one involving use of an improvised explosive device against a boat. It is thought to be the first such attack since the occupation of Iraq began in 2003. There is no indication at this point as to which service branch the victims belonged to.

Sunday, November 12, 2006 12:18 PM - The Slovakian Ministry of Defense has confirmed the death of Rotný (Sergeant 1st Class) Rastislav Neplech in Iraq on Saturday, November 11th. In addition, an article (also in the Slovak language) is to be found at a publication called SME. It would appear that Neplech was born in the town of Žilina in north central Slovakia in 1974, making him 32 years old. However, he lived in the town of Vrútky. He was attached to a rapid deployment battalion (prápore rýchleho nasadenia) stationed in the town of Martin not far from Vrútky. Neplech was married and had two children.

Sunday, November 12, 2006 6:45 PM - The Livingston, Michigan, Daily Press & Argus is reporting the death of a soldier who grew up in Livingston County in the town of Webberville: Staff Sergeant Gregory McCoy, 26. According to the article, McCoy died in Baghdad on Thursday, November 9th, when an improvised explosive device detonated, sending his vehicle off of an overpass. He was with the 410th Military Police Company of the 89th Military Police Brigade based out of Fort Hood, Texas, making him one of the two deaths described in this CENTCOM release.

Monday, November 13, 2006 2:54 AM - The British Ministry of Defense has not yet released the names of the four service members who died in an IED attack on their patrol boat on the Shatt Al-Arab waterway on Sunday, November 12th ... but they have now identified their units. Two were British Army: one from the Royal Corps of Signals, one from the Intelligence Corps. The other two deaths were British Royal Marines.

Monday, November 13, 2006 5:46 AM - Both the Associated Press and the Detroit Free Press are reporting the death of a Wayne County, Michigan, Marine in Iraq. Sergeant Bryan Burgess, 35, was apparently killed instantly in Fallujah on Thursday, November 9th, when struck in the face by a sniper's bullet. He was a Marine reservist based at Selfridge Air National Guard Base near Mount Clemens, Michigan. This death has not previously been reported by CENTCOM. Incidentally, Burgess was the second Marine reservist to die by sniper fire that day. Marine Cody Warren was the first that we learned of.

Monday, November 13, 2006 6:20 AM - An English language media outlet called the Slovak Spectator has posted an article that provides more detail on the incident that killed Slovak soldier Rastislav Neplech on Saturday, November 11th. According to it, Neplech was in the third vehicle of a convoy composed of 10 Slovak, four U.S. and six Salvadoran vehicles. The ambush occurred on a narrow street in a built up area of Al Kut. Incidentally, the article states that Neplech was a Private, but the Slovak Ministry of Defense has already confirmed that he was a Sergeant 1st Class.

Monday, November 13, 2006 6:37 AM - CENTCOM is reporting the deaths of two soldiers assigned to the 3rd Brigade Combat Team, 82nd Airborne Division. They were killed on Sunday, November 12th, when their vehicle was hit by a suicide car bomb in Salah ad Din Province.

Monday, November 13, 2006 9:29 AM - On November 10th, MNF-Iraq issued this release stating that a 13th Sustainment Command soldier was killed west of Haditha on November 10th. Shortly thereafter, CENTCOM issued the same release, only with the date of death changed to Thursday, November 9th. On November 11th, the Associated Press and the Ontario Daily Bulletin reported the death of "Army Reserve" Sergeant 1st Class Rudy Salcido on "Wednesday", November 8th, while serving in Al Asad, a large air base south of Haditha. Finally, the DoD has now confirmed Salcido's death ... on Thursday, November 9th ... in Baghdad ... stating that he was a California Army National Guardsman. At this point, it seems safe to accept November 9th as the actual date of death, especially since the day cited by the media could have been stateside time, not Iraq time. But what to make of his place of death? We know from family statements that he was serving at Al Asad. On September 30th, 2006, there were two other 13th Sustainment Command deaths, Staff Sergeant Nisely and Specialist Sourivong, both National Guardsmen ... both dead from small arms fire near Al Asad according to CENTCOM and the DoD. So ANG troops attached to the 13th Sustainment Command certainly did have a presence at Al Asad. Until and unless further information comes to light, we are going to assume that Sergeant Salcido died west of Haditha per the CENTCOM report ... not in Baghdad. Salcido was 31 and from Ontario, California.

Monday, November 13, 2006 10:29 AM - WMTW News of Maine is reporting the death of a soldier who grew up in Thomaston, Maine: Sergeant William Samuel Jackson II, 29. According to the article, Jackson died at Ar Ramadi when an improvised explosive device detonated on Saturday, November 11th. He is likely one of the three 1st Armored Division deaths reported in this CENTCOM release.

Tuesday, November 14, 2006 10:31 AM - The Polish Ministry of Defense has issued a notice that the funeral of Sergeant Tomasz Murkowsky, killed in Iraq on November 11th, will take place in Elblag, Poland, on Wednesday, November 15th. Roughly translated, the article provides a list of dignitaries who will attend the ceremony, including the Polish president and the defense minister. A funeral mass is scheduled to begin at the cathedral in Elblag at 11 AM. The coffin will then be carried in a solemn procession to the municipal cemetery on Agrykoli Street where the burial ceremony will begin at about 1 PM.

(1) MNF-Iraq is reporting the deaths of two more soldiers on Tuesday, November 14th. They died when their vehicle was struck by a roadside bomb in northwest Baghdad.
 
(2) According to yesterday's DoD release confirming the deaths of 1st Lieutenant Michael Cerrone and Private 1st Class Harry Winkler on Sunday, November 12th, both men were from Clarksville, Tennessee. However, the DoD would appear to be in error here. The Fayetteville Observer has an article out on both men that identifies Winkler's hometown as Jacksonville, Florida. The article also reports that Cerrone was the son of Brigadier General James Cerrone who is the special assistant to the commanding general of the 18th Airborne Corps and Fort Bragg.
 
(3) The Sydney (Australia) Morning Herald is reporting that Corporal Ben Nowak, one of the British Royal Marines who died in Iraq on Sunday, November 12th, actually had dual Australian-British citizenship. His mother lives in Erskine in Western Australia. Nowak was in Iraq for only one week when an IED detonated near his patrol boat on the Shatt Al-Arab, killing him.
 
(4) This morning, the Polish Ministry of Defense published a longer article on Sergeant Tomasz Murkowski, who died in a roadside bomb blast in Al Kut on Saturday, November 11th. It would appear that his hometown is Elblag, Poland ... the same city in which he will be buried today with full military honors. He has also been posthumously promoted to "Starzy Sierżant", Sergeant 1st Class.

(2) On November 10th, both CENTCOM and MNF-Iraq issued identical press releases stating that a Regimental Combat Team 7 Marine had died "today" from non-hostile causes in the Al Anbar Province. This death was duly entered into our database on the 10th. Also on November 10th, an article in the Calhoun Times reported the death of 19 year old Cody Warren, a Marine reservist, in Iraq on November 9th. The article stated that "friends of the Marine said he was killed by sniper fire". So a new entry was made in our database for this Marine on the 9th. Now, the DoD has formally confirmed the death of Lance Corporal Kristopher Cody Warren ... on November 9th ... "from a non-hostile incident". The CENTCOM releases just cited mark the only non-hostile death since November 6th, indicating that that death and Warren's are most likely one and the same. Once again, it would appear that we have a case of CENTCOM preparing a release when "today" was the 9th, but not publishing it until the 10th. As for the rumor of Warren's death being from sniper fire, there actually was a Marine reservist killed from sniper fire on that day: Sergeant Bryan Burgess. Our guess is that the two deaths were confused when the families were first notified. Regardless, the nameless non-hostile death that appeared in our database on the 10th has been eliminated, reducing the death count by one.

The article indicates that he was a graduate of the University of Richmond, that his funeral might take place there, and that burial is planned at a National Cemetery near Sandston, Virginia. Wednesday, November 22, 2006 1:36 PM - The DoD has confirmed the death in Iraq of Specialist Eric Vizcaino, 21, of New Mexico. It would appear that earlier media reports were incorrect in several particulars. He did not die on Monday, November 20th, as reported, but was injured that day in a non-combat incident in Samarra. Vizcaino actually died in a medical facility in Balad, Salah ad Din Province, on Tuesday, the 21st. The nature of the incident that cost him his life is not yet clear, although media accounts consistently describe a shooting of some sort. What is clear is that Vizcaino is the non-hostile death described in this CENTCOM release. That non-hostile death has now been merged with Vizcaino's, reducing the death count by one.

(2) The Associated Press is reporting the crash of a U.S. Air Force jet in Iraq near the town of Karmah in Al Anbar Province on Monday, September 27th. The jet was apparently providing close air support to troops on the ground. An Iraqi who witnessed the crash was quoted as saying that the jet was flying erratically before it "nosedived and exploded in a farm field". The U.S. military has made no statement as of yet as to the cause of the crash or the fate of the pilot.

According to their report, the craft was engaged in support of combat operations on the ground ... and went down "with one pilot on board". For this reason, we are removing him from the database until official word of his death is received.
 
(2) The Danbury, Connecticut, News Times is reporting the death of an army soldier born in New Haven: Captain Jason Hamill, 31. Saturday, December 02, 2006 9:05 AM - The Associated Press is reporting the death of an army soldier who grew up near Omaha, Nebraska. Staff Sergeant Jeremy W. Mulhair, 35, was killed near Taji, just northwest of Baghdad, when a roadside bomb exploded on Thursday, November 30th.

Saturday, December 02, 2006 10:31 AM - The DoD has confirmed the death of Staff Sergeant Jeremy W. Mulhair, 35, of Omaha, Nebraska, on Thursday, November 30th. He died in Taji (northwest of Baghdad) when a roadside bomb struck his vehicle.

Sunday, December 03, 2006 8:27 AM - The DoD has completed DNA testing of samples taken from the site of the crash of an F-16C fighter jet near Fallujah on November 27th and are now officially declaring the pilot dead. He was Major Troy L. Gilbert, 34, of Litchfield Park, Arizona ... a member of the U.S. Air Force based at Luke Air Force Base in Arizona. There is still no word from the military on whether the crash was caused by hostile or non-hostile means.

Monday, December 04, 2006 4:24 PM - The DoD has released the identity of the soldier who died in Baghdad from enemy action on Thursday, November 30th: Staff Sergeant John L. Hartman Jr., 39, of Tampa, Florida. He was killed when an improvised explosive device detonated near his vehicle.

Wednesday, March 21, 2007 5:45 PM - Five months after the fact, the DoD has announced the death of Lieutenant Colonel Peter E. Winston, 56, of Plant City, FL. He is reported to have died on November 13, 2006, in Kaiserslautern, Germany, from a non-hostile unspecified cause that occurred while he was stationed in Iraq.

Monday, March 26, 2007 7:52 PM - The Associated Press is quoting a spokesman for the 143rd Sustainment Command in Orlando, FL, as saying that Army Reservist Lieutenant Colonel Peter Winston, 56, of Plant City, Florida, had committed suicide. Winston, a long time member of the Florida National Guard and Army Reserve, had volunteered for active duty four years ago and had served in Kuwait as a safety officer. According to information gleaned from the DoD casualty databases, his death occurred at Rhine Ordnance Barracks adjacent to Ramstein Air Base in Kaiserslautern, Germany, on November 13, 2006.
